If it is a year old i would get a new bottle especially if it has been used before. If your at has not been on this insulin in over a year it can be unsafe to switch insulins or start again without a proper glucose reading

I would not recommend re-using it. Once the ampule is broken, the contents are no longer considered sterile, so using them in an infection the next day, with an unsealed container, increases risk for an abscess (bacterial infection ) at the injection site). If the injections aren't helping much, talk to your vet about whether or not its worth it to continue doing them. Your vet may recommend rechecking bloodwork to see where those kidney values are at; there may be improvement that you can't appreciate on the outside, but the values may be better which would indicate that continuing the injections is wise. You can feed the royal canin indefinitely. It is safe long term. However it is ok to give some other food occasionally as well, especially if he's not wanting to eat the renal diet. Some cats prefer canned food so you may try the canned version if you haven't tried that yet. Thanks for asking Boop by Petco!

This is from the manufacturer's website with regards to storing Eprex: Store at 2°C to 8°C. Do not freeze or shake. This temperature range should be closely maintained until administration to the patient. Store in original package in order to protect from light. When the product is about to be used, it may be removed from the refrigerator and stored at room temperature (below 25°C) for a maximum single period of seven days. So, if you keep it refrigerated at 2-8C even after opened, it should be OK to use. You could always call the manufacturer (phone number should be on the packaging) to get better council.

If you live near a Petco that has a full service hospital then yes, you could talk to the vet about refilling your cat’s prescription. If your Petco has a Vetco vaccine clinic then the vet performs vaccines and routine preventative care only and would not be able to authorize refills.
